Approval by the Board of Education of Amendment No. 5, Independent Consultant Agreement between the District and Anthonio, Inc., Oakland, CA, for unchanged Scope of Work, Inspector of Record services, based on 12 additional weeks of work, as described in the attached Proposal dated February 10, 2021, in an additional amount of $22,195.00, increasing Agreement not to exceed amount from $1,289,157.00 to $1,311,352.00, extending term of Agreement from January 13, 2016 through December 4, 2020 to May 31, 2021 (an additional 178 calendar days), and authorizing the President and Secretary of the Board to sign the Amendment for same with said Consultant. All other terms and conditions of the Agreement remain in full force and effect.
