Ratification by the Board of Education of a Professional Service Contract between District and Genevieve Reis, Albany, CA, for the latter to a served as a License Clinical Social Worker (LCSW) Supervisor to perform the following tasks: 1.Coimplete invoices to bill for clinical services provided under Medi-Cal Contract with Alameda County Behavioral Health Care Services; 2. Provide LCSW supervision of clinicians who perform psychiatric social work services in assigned Mental Health Special Day classrooms on site and in the field; 3. Maintain and gather confidential client records per Health Insurance Portability and Accountability Act (HIPPA); 4. Facilitate individual, group and milieu therapy services training as the supervising LCSW; and 5. Prepare mental health chars for required Clinical Quality Review per Medi-Cal and Alameda County Mental Health Contract requirements, for Programs for Exceptional Children, for the period of October 14, 2013 through March 15, 2014, in an amount not to exceed $10,000.00.
