Title: Approval by the Board of Education of Amendment No. 1, Professional Services Contract between District and Hatchuel Tabernik & Associates, Berkeley, CA, for the latter to 1) provide an additional 24 gang prevention workshops for OUSD school staff and partner agencies: a) a total of 80 hours of training services shall be provided; b) pre-post surveys shall be administered; c) workshops will be scheduled and coordinated under the supervision of OUSD's Office of Alternative Education; 2) participate in quarterly Interagency Gang Prevention Collaborative with work groups; 3) provide data as required for evaluation; and 4) provide 70 hours of project sustainability activities including grant reporting, writing, and evaluation, for the period October 1, 2012 through June 30, 2013, in an additional amount not to exceed $15,000.00, increasing the Contract not to exceed amount from $4,000.00 to $19,000.00. All other terms and conditions of the Contract remain in full force and effect.
