Title: Approval by the Board of Education of Amendment No. 4, Independent Consultant Agreement for Professional Services between District and Michael Willis Associates, Oakland, CA, t is to balance the account with latter due to commingling of funds with another project in conjunction with the Roosevelt Middle School Health Center Project, increasing the Contract not to exceed amount by $6,866.39, from $101,708.00 to $108,574.39, for extending the contract period of June 30, 2010 through December 31, 2011 to December 31, 2012. All other terms and conditions of the Contract remain in full force and effect.
