Title: Ratification by the Board of Education of Amendment No. 1 of a Professional Services Contract between District and Jazmine Chavez, Oakland, CA, for the latter to provide an additional 62 hours for consultant to daily teach second language learners in the computer lab using the Mind Institute and Grow Connection curriculums; meet with parents to translate both goals and individual student progress for both curriculums; track student progress via biweekly testing to ensure that adequate academic progress is made by participating students at Reach Academy and extending the contract period for an additional 11 days, from the period December 7, 2009 to June 2, 2010 to June 17, 2010, for an additional amount of $928.00, increasing the original contract not to exceed amount from $2,669.50 to $3,597.50. All other terms and conditions of the Contract remain in full force and effect.
