Title: Approval by the Board of Education of Amendment No. 5, General Services Agreement by and between the District and Terraphase Engineering Inc., Oakland, CA, for the latter to provide additional environmental services, including the installation of three additional soil vapor probes and sampling of the new and existing probes, as recommended by the Department of Toxic Substances Control (DTSC), as outlined in the Proposal dated May 13, 2026, which is attached to this Amendment as Exhibit A, for the Garfield Elementary School Modernization Project, in an additional amount of $43,033.00, which includes a contingency fee of $3,707.00, increasing the Agreement not-to-exceed amount from $120,797.00 to $163,830.00, and extending the term of the Agreement from May 15, 2025, through December 31, 2026 to June 30, 2027 (an additional 184 days). All other terms and conditions of the Agreement remain in full force and effect.
