Ratification by the Board of Education of a Professional Services Contract between the District and Victoria King, Hayward, CA, for the latter to lead the site-based development of family resources; case manage high risk families and provide linkages to basic resources; develop case management resources; coordinate an integrated quality family support program; conduct home visits with families of students participating in an extended day component; participate in site-based services coordination process; act as advocate liaison between high risk student families and legal guardians and various community program services and provide other services, as described in the Scope of Work, incorporated herein by reference as though fully set forth, at Madison Academy, for the period November 2, 2015 through June 9, 2016, in an amount not to exceed $9,999.00.
