Title: Ratification by the Board of Education of a Professional Services Contract between District and Peter Mummert, San Francisco, CA, for the latter to work collaboratively with site and school leadership to communicate to 100% of the qualifying high school students the services offered by the SES Program (i.e., tutoring free of charge); to monitor participating students monthly program and school attendance; to maintain student attendance data for 100% students who are receiving services for year-to-year comparison; to establish communications and a creative tool to provide 100% of the SES providers with information that will help build partnerships with community health service providers so that at least 80% of students with health concerns are referred to the District's Health Services or appropriate community health provider, for the period of July 1, 2011 through June 30, 2012, in an amount not to exceed $68,000.00
