Title: Ratification by the Board of Education of a Professional Services Contract between the District and Martin L. Forst, Oakland, CA, for the latter to provide 290 service hours to work as an Evaluator of the Tobacco Usage Prevention Education High School (TUPE-HS ), Second Step-K-5, and Caring School Community (CSC) Programs; devise student survey forms; for quit and awareness groups; year end student survey; input all project reporting forms for analysis, and send out a monthly report/update to the Project Director; design evaluation of the Second Step preschool and elementary programs, and to design evaluation of the CSC program, which is beginning to be implemented this year in 11 elementary sites; produce separate evaluations of each of the two programs, for the period of November 1, 2009 through June 30, 2010, in an amount not to exceed $14,500.00.
