Title: Ratification by the Board of Education of Amendment No. 1 of Professional Services Contract between the District and Patricia (Patty) Garcia, Crockett, CA, for the latter to provide an additional 418.91 hours of service conducting mandatory smokeless school day - tobacco awareness groups for students caught with tobacco; conduct TAP quit cessation groups and counseling for students attempting to quite using tobacco; provide intake and assessment of students caught with tobacco; fill out a monthly activities log and providing that information to Tobacco Usage Prevention Education evaluator and act as program site coordinator for six to eight school sites, for the period of September 14, 2009 through June 30, 2010, for an additional amount not to exceed $15,500, increasing the original contract not to exceed amount from $20,000 to $35,500. All other terms and conditions of the contract remain in full force and effect.
