Title: Ratification by Board of Education of Professional Services Contract between District and Mary Jonlick, RN, for the latter to serve as District's diabetes consultant, under supervision of the Program Manager for Health Services, working with team leaders to administer insulin injections to students pursuant to MD instructions, check said pupil's blood glucose levels and perform other duties, as described in the Scope of Work and incorporated herein by reference as though fully set forth, at the District's Health Services Department, for the period of August 28, 2008 through June 30, 2009 at a cost not to exceed $17,000.00.
