Title: Approval by the Board of Education of a Professional Services Contract between the District and California Youth Outreach (CYO)-Oakland, Inc., Oakland, CA, for the latter to provide the following services as part of the Measure Z Juvenile Justice Wraparound Grant: facilitate Re-Entry Welcome Care for youth clients at alternative schools using Restorative Justice Principles; create service plans including goals and action plans through Multidisciplinary Team process for the youth clients as well as provide case Management for youth through the end of the 2015-2016 school year including multiple hours of case management support and case management consisting of home visits, individual meetings, phone calls, crisis intervention, and referral services.; and provide Gang Redirect Life Skill classes as well as create life plans and goals for each youth client, for Schools Network No. 4, for the term February 1, 2016 through June 30, 2016, in an amount not to exceed $30,000.00.
