Ratification by the Board of Education of Amendment No. 1, Professional Services Contract between the District and Rhana Hashemi, Berkeley, CA, for the latter to provide an additional 660 hours of classroom and assembly anti-tobacco/marijuana presentations, Alcohol, Tobacco & Other Drugs (ATOD) awareness sessions for students violating District tobacco/drug free policies, cessation and strengths-based life skills and behavior coaching at 10 plus middle and high school sites; abide by Behavioral Health policies and procedures, as described; and provide or perform other services, as outlined in the Scope of Work, incorporated herein by reference as though fully set forth, for the additional amount of $16,000.00, increasing the Contract not to exceed amount from $16,000.00 to $37,780.00 and extending term of Contract from August 21, 2017 through December 19, 2017 to June 30, 2018. All other terms and conditions of the Contract remain in full force and effect.
